Dear Patient,

Thank you for choosing Orthopaedic Center of Southern Illinois.  It is our desire to provide for your healthcare needs both thoroughly and efficiently.  The initial appointment is spent conducting an examination and discussing treatment plan options.

In order to provide a more detailed and informative exam, we ask that you complete the attached form(s) prior to your appointment.   If you have received this packet and have been a past patient, it is still necessary for you to complete these forms in order for us to update your information.

In order to expedite the registration process, it is very important that you arrive 15 minutes prior to your appointment time with your completed forms.

Please note that you will be responsible for any co-payment, co-insurance, or deductible amounts as set forth by your insurance company.  We will verify your insurance benefits and deductible amounts prior to your appointment.  To avoid your appointment being rescheduled, if you have any intention of filing a worker’s comp or liability claim, please do so prior to your appointment and notify our office in order for us to obtain proper approval.

Please bring a CD of the images and a report of all current x-rays, MRI, CT Scans, Bone Scans and or Dexascan.  If you have not had recent imaging, please disregard.  If this information is not brought to your appointment, your appointment may be cancelled. Please also bring a list of current medications. 

Every effort will be put forth to keep your waiting time to a minimum; however, emergencies do arise. When this happens, your patience is appreciated, and you will be informed of the delay.
