In order to expedite the registration process, it is very important that you arrive 15 minutes prior to your appointment time with your completed forms. If you have any intention of filing a worker's compensation or liablity claim, please do so prior to your appointment and notify our office in order for us to obtain proper approval and to avoid having your appointment rescheduled.
Upon checking in for your appointment, you will be greeted by our friendly Reception staff whose goal is to make your registration process as easy and as fast as possible. We take pride in being attentive to your needs and keeping you as comfortable as possible during your wait.
Upon check out, our highly trained Receptionists will assure that you have all necessary follow up appointments scheduled and will make sure that all of your questions have been answered.
